Related to increased weight and pressure in the affected area, impacting movement. Related to reduced muscle flexibility and joint stiffness. Interventions The school nurse will assess the level of swelling and range of motion in affected areas regularly. The school nurse will educate on and encourage light exercises or stretches that promote lymphatic drainage, as recommended by physical or occupational therapists. The school nurse will collaborate with a physical therapist to develop a personalized exercise plan that improves mobility while reducing lymphedema. The school nurse will provide information on the use of compression garments and assist in applying them correctly, if needed. The school nurse will encourage elevating the affected limb(s) when at rest to promote fluid return. Expected Outcomes The student will demonstrate improved mobility in the affected area with reduced stiffness and discomfort. The student will actively participate in exercises and other mobility-promoting activities. The student will report decreased heaviness and improved comfort in the affected limb(s). Nursing Diagnosis: Risk for Impaired Skin Integrity Related to skin stretching and fragility due to chronic swelling. Related to increased risk of infection and delayed healing in affected areas. Related to possible breakdown due to poorly fitting clothing or injuries to the edematous limb(s). Interventions The school nurse will assess the skin for signs of breakdown, redness, dryness, or infection daily. The school nurse will educate on gentle skincare, including moisturizing and using mild soaps to prevent dryness and cracking. The school nurse will instruct on safe use of compression garments to avoid excessive pressure and ensure proper fit. The school nurse will encourage avoiding tight or restrictive clothing and accessories on the affected limb(s). The school nurse will teach the importance of infection prevention and self-monitoring for any early signs of infection. Expected Outcomes The student will maintain intact skin in the affected areas with no signs of breakdown or infection. The student will demonstrate appropriate skin care practices, including regular moisturizing. The student will report understanding of infection prevention and avoid skin injuries in the edematous area. Nursing Diagnosis: Risk for Infection Related to reduced immune response in the affected limb(s) due to impaired lymph drainage. Related to increased risk of cuts, abrasions, or skin breakdown due to stretched and fragile skin. Related to limited mobility, leading to areas of the body not receiving sufficient cleansing and care. Interventions The school nurse will teach proper hygiene for the affected area, including gentle cleaning and drying. The school nurse will encourage using protective measures, such as gloves during activities that risk skin injuries (e.g., sports or certain arts and crafts). The school nurse will educate the student on early signs of infection (e.g., redness, warmth, swelling, fever) and the importance of reporting symptoms immediately. The school nurse will promote the regular use of compression garments as prescribed, which can help control swelling and reduce skin damage. The school nurse will collaborate with healthcare providers for routine skin and infection checks, especially if swelling persists or worsens. Expected Outcomes The student will remain free from infection, as evidenced by normal skin appearance and no signs of inflammation or redness. The student will understand and use protective strategies to prevent skin injuries. The student will report any signs of infection promptly, allowing for early intervention. Nursing Diagnosis: Disturbed Body Image Related to visible swelling or changes in body contour due to fluid accumulation. Related to self-consciousness about the appearance of the affected limb(s) and use of compression garments. Related to limited activity or participation in social and recreational activities due to discomfort or embarrassment. Interventions The school nurse will encourage open discussions about body image concerns and provide a supportive, nonjudgmental environment. The school nurse will offer counseling or refer to a support group for students with chronic conditions, providing an opportunity to share experiences and coping strategies. The school nurse will reinforce the student’s strengths, abilities, and qualities beyond their physical appearance. The school nurse will provide information on clothing or accessories that can help camouflage swelling, if desired by the student. The school nurse will educate teachers and classmates about lymphedema (if the student consents) to foster an inclusive and understanding environment. Expected Outcomes The student will demonstrate improved self-acceptance and express positive aspects of self-image. The student will engage in social and recreational activities with greater confidence. The student will report feeling less self-conscious about their condition and appearance. Nursing Diagnosis: Activity Intolerance Related to increased physical and mental effort required for daily tasks due to swelling and discomfort. Related to reduced stamina or endurance from fatigue associated with chronic lymphedema. Related to potential need for rest breaks due to heaviness and limitations in the affected limb(s). Interventions The school nurse will assess the student’s tolerance for physical activities, noting any signs of fatigue or discomfort. The school nurse will collaborate with physical therapy to create a balanced activity program that includes both movement and rest. The school nurse will encourage pacing techniques and rest periods during physical tasks, especially if swelling or discomfort worsens. The school nurse will teach energy conservation techniques, such as breaking tasks into smaller steps or using assistive devices. The school nurse will monitor the student’s response to activities and make adjustments to avoid overexertion. Expected Outcomes The student will demonstrate improved tolerance for physical activities by using rest breaks and pacing. The student will report reduced fatigue and greater comfort when engaging in daily tasks. The student will participate in school activities to the fullest extent possible within their limitations. Nursing Diagnosis: Knowledge Deficit Related to limited understanding of the chronic nature of lymphedema and its management. Related to need for education on self-care practices, including skincare, exercise, and symptom monitoring. Related to lack of awareness of potential complications, such as infection or worsening edema. Interventions The school nurse will educate the student and family about the causes, symptoms, and progression of lymphedema. The school nurse will teach techniques for self-management, such as applying compression garments, gentle exercise, and elevating the affected limb(s). The school nurse will provide resources on lymphedema care, including handouts or access to lymphedema specialists. The school nurse will encourage self-monitoring for signs of complications (e.g., sudden swelling, skin changes) and knowing when to seek medical help. The school nurse will ensure the student understands how to incorporate lifestyle changes, such as protective clothing and gentle skincare, into their daily routine. Expected Outcomes The student will demonstrate an understanding of lymphedema management, including self-care practices. The student will actively monitor their symptoms and report any changes or complications promptly. The student will experience fewer complications and greater control over their condition.
